The Collaboratory at Albany College of Pharmacy and Health Sciences is a community-based hub created to improve health outcomes and reduce disparities in Albany’s South End.
Improving Community Health and Wellness in Albany’s South End
The Collaboratory: Your Neighborhood Health Clinic
The Collaboratory at ACPHS is a community-based hub located at 200 South Pearl Street, just two miles from campus, created to improve health outcomes and reduce disparities in Albany’s South End.
Signature services are delivered by ACPHS’ Public Health Pharmacy Team (PHPT) and Trinity Alliance’s Wellness Advocates Linking Communities (WALC) team.
By bringing together ACPHS students, faculty and local partner organizations, The Collaboratory delivers innovative, people-centered care while serving as a hands-on learning environment for the next generation of health professionals.
Our mission is to reduce health disparities by harnessing the collective expertise of ACPHS students, faculty and local partner organizations.
What We Offer
Educational Programs
We provide learning opportunities for both South End residents and ACPHS students that empower people with knowledge to take control of their health.
Topics include: asthma management, smoking cessation,
medication literacy.
These programs are designed to boost health literacy and directly address disparities that impact chronic health conditions in the community.
Community Health Services
The Collaboratory offers a wide range of health and wellness services, free of charge and tailored to local needs: food assistance, health-insurance navigation, health and medication literacy, chronic disease self-management programs, HIV adherence monitoring, housing and shelter support, medication reviews, individual and family support.
Our approach is holistic and community-first, removing common barriers to care while building trust through a consistent local presence.
A Space to Learn, Meet & Collaborate
The Collaboratory also serves as a meeting and event space for South End residents and organizations. Local partners including AVillage, South End Community Collaborative and BirthNet have used the space for meetings and outreach efforts.
We regularly host: educational workshops, cultural events, community planning sessions and wellness initiatives.
Empowering Students with Real-World Experience
The Collaboratory is more than a community resource—it’s a vital part of the ACPHS educational experience. Public Health students collaborate with faculty and community health workers to address real health-care challenges, gaining essential skills in problem-solving, teamwork and public health practice.
The Public Health program held its first course at The Collaboratory in Fall 2018, and ACPHS continues to expand student learning opportunities on-site, including educational workshops focused on chronic-disease prevention and health literacy.
Getting there is easy—The Collaboratory is just two miles from the main ACPHS campus, with free transportation via CDTA buses and ample on-street parking for those who drive.
